    The goods and services shown below are those we can provide for our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for basic services and overhead. If legal or other requirements mean you must buy items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.


    PROFESSIONAL SERVICES:

    Basic services of Funeral Director and Staff and Overhead $895.00
    Our services include: conducting the arrangement conference, planning the funeral; consulting with family and clergy; shelter of remains; preparing and filing of necessary notices; obtaining necessary authorizations and permits; coordinating with the cemetery, crematory, or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs.

    This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

    Other Preparation and Care for Private Viewing $300.00
    This includes necessary washing and disinfection, and preparation of body for viewing on dressing table or in the alternative container.

    Direct Cremation $895.00
    Our packaged charge for direct cremation (without ceremony) includes: basic services of funeral director and staff, a proportionate share of overhead costs, removal of remains within 70 miles of the twin cities, transportation to crematory, necessary authorizations, minimum cremation container, medical examiner fees, and the crematory fee.

    If you want to arrange a cremation, you can use an alternative container. MN Statute 149a does not require you to purchase a casket for cremation. A cremation container is a combustible, closed container resistant to the leakage of bodily fluids, that encase the body and can be made of materials like fiberboard or composite materials (with or without an outside covering). The container we provide, at no cost to you and included in the packaged price is cardboard.

  • Understanding of the Right and Duty to Control the Final Disposition of the Deceased

    Minnesota Stat. § 149A.80 subd. 2 sets forth the individuals who have the right and duty to control the final disposition of a decedent's remains. The individual(s) who have a right and duty to control the final disposition of a decedent's remains are, in descending order:

    • The person or persons appointed by the decedent IF the decedent has properly executed an advance directive under Minn. Stat. §149A.80 subd. 1;
    • current and legal spouse of the decedent;
    • the adult child of the decedent, who must certify that they represent the wishes of the majority of the adult children and have no actual knowledge of an objection to the contrary regarding the final disposition by any adult child of the decedent;
    • the surviving parent or parents of the decedent, each having equal authority;
    • the adult sibling of the decedent, who must certify that they represent the wishes of the majority of the adult siblings and have no actual knowledge of an objection to the contrary regarding the final disposition by any adult sibling of the decedent;
    • the adult grandchild or the majority of the adult grandchildren of the decedent, provided that, in the absence of actual knowledge to the contrary that the adult grandchild or grandchildren represent they are the only grandchild or grandchildren reasonably available to control the final disposition of the decedent;
    • the grandparent or grandparents of the decedent, each having equal authority;
    • the adult nieces and nephews of the decedent, provided that, in the absence of actual knowledge to the contrary that the adult nieces and nephews represent they are the only nieces and nephews reasonably available to control the final disposition of the decedent;
    • the person(s) who were acting as the guardians of the decedent who also had legal authority to make health care decisions for the decedent at the time of death;
    • an adult who exhibited special care and concern for the decedent;
    • the person(s) respectively in the next degree of kinship in the order named by law to inherit the estate of the decedent;
    • the appropriate public court or authority, as required by law.

    The person(s) who are of the highest level in this order have the right and duty to control the final disposition of the decedent.

  • Crematory Authorization Form

  • This authorization form is to comply with the cremation requirements of Minnesota Statutes §149A.95, subd. 4. This form grants permission to Monarch Crematory Funeral Establishment to cremate a dead human body. The person(s) signing this document declare(s) authority to control the final disposition of the deceased person named below in accordance with Minnesota Statutes §149A.80.

    The person(s) signing this form below make(s) the following statements and/or acknowledge(s) being advised of the following:

    • I request and authorize Monarch Crematory Funeral Establishment, located in Bloomington, Minnesota, a sub-contractor of ecremationmn.com, to cremate the human remains of who died on and was born on in accordance with all applicable laws of the State of Minnesota.
    • I have legal control to authorize the final disposition and cremation of the deceased person named above.
    • To the best of my knowledge, I attest that the body of the deceased named above does not contain an implanted mechanical or radioactive device, such as a heart pacemaker, that may create a hazard when placed in the cremation chamber. If a device is implanted, I authorize the device to be removed per Minnesota Statute §149A.95, subd. 7.
    • I authorize Monarch Crematory Funeral Establishment to remove the body from the container in which it was delivered, if that containeris not appropriate for cremation, and to place the body in an appropriate cremation container. The crematory named above may dispose of the original container in a lawful manner as they see fit.
    • I understand that under Minnesota Statute §149A.95 subd. 5, Monarch Crematory Funeral Establishment may reasonably rely upon this authorization to cremate and that I shall hold it harmless from civil liability or criminal prosecution for any lawful actions performed by the above named crematory.
    • I authorize Monarch Crematory Funeral Establishment to open the cremation chamber and reposition the body to facilitate a thorough cremation and to remove from the cremation chamber and separate from the cremated remains, any noncombustible materials or items. The crematory may dispose of any noncombustible materials or items in any lawful manner as they see fit, unless specific instructions are attached to this form.
    • I acknowledge that the cremated remains will be mechanically reduced to a granulated appearance and placed in an appropriate container. I authorize Monarch Crematory Funeral Establishment to place any cremated remains that a selected urn or container will not accommodate into a temporary container to be disposed of in the same manner as the original container as noted below in (8).
    • I acknowledge that, even with the exercise of reasonable care, it is not possible to recover all particles of the cremated remains and that some particles may inadvertently become commingled with disintegrated chamber material and particles of other cremated remains that remain in the cremation chamber or other mechanical devices used to process the cremated remains.
    • I direct Monarch Crematory Funeral Establishment to release the cremated remains in the following manner: Return to the funeral home in a plastic container.

  • Basic Information on the Deceased

    This information will be used to complete a Minnesota State Death Certificate. There are two types of Death Certificates: Fact of Death and Cause of Death. The only thing one needs a Cause of Death Certificate for is life insurance claims. Everything else one would need a death certificate for can be accomplished with the Fact of Death Certificate.

    Below is a list of some of the places that will ask for a Fact of Death Certificate:

    Real Estate Title transfers
    Title transfer of cars, boats, motorcycles, et cetra
    Canceling of contracts such as Cell phones, cable, and internet
    Mutual funds
    Probate court

    With the filling of the vital statistic information, the Social Security Administration will also be notified and social security benefits will be adjusted accordingly.
